School management plans
If you need a new or updated school management plan for your child, you can email the Diabetes Educators for help to complete the plan. Send your message to: ISLHD-NurseDS@health.nsw.gov.au
Please include your child's name and date of birth in the subject heading of the email. We will respond to your email within 7 days.
We encourage you to start a management plan which you feel is most suitable to your child's needs. The following websites have examples of a management plan template which you can download. There are also other useful resources to support managing diabetes in schools
